Player Responsibilities: I understand that as a member of Ambush that I am expected to behave in an appropriate and responsible manner; I understand that I will be held accountable for my actions. - Display good sportsmanship in victory and defeat;
- Avoid fighting and/or excessive physical contact before, during, and after games/practices;
- Refrain from using vulgar/obscene language to teammates, coaches, opponents, officials, and fans/spectators;
- Treat facilities and equipment being utilized with care and respect;
- Wear appropriate team uniform in the appropriate manner; refrain from wearing jewelry or other inappropriate items;
- Treat my coaches with respect and notify them in a timely fashion if I can not attend practice or games;
- Be a good Teammate!
Parent/Guardian Responsibilities: I understand that as a parent of an Ambush Athlete that I am expected to behave in an appropriate and responsible manner; I understand that I will be held accountable for my actions. I understand that if I do not behave in an appropriate and responsible manner at games that I may be ejected from the facility and barred from attending games. To that end, I will…
- Help my child achieve items #1 through #7 listed above by modeling the appropriate behaviors;
- Behave rationally and responsibly towards all players, coaches, officials, site directors, etc., even if I’m not sure others are behaving accordingly;
- Focus on learning and enjoyment more than winning and losing;
- Think about more than just what is good for my child;
- Discuss concerns with coaches at an appropriate time and place and in an appropriate manner (not immediately after a game or before a practice is about to begin);
- Extend common courtesy to everyone involved in our program;
- Avoid undermining team morale;
- Parents are responsible for dropping off and picking up players for practice and games on time.
- Parents should refrain from coaching from the sideline.
- Reinforce the coach’s instructions while helping my child develop his personal skills;
- Practice positive reinforcement whenever possible.
In accordance with the Ambush code of conduct above, I understand that I, as a player and/or a parent, am responsible for my actions/my child’s actions as they are presented in this document. I understand that the program has the right to suspend and/or dismiss (or even bar from various facilities) coaches, players, or parents for conduct/behavior that it deems inappropriate or detrimental to the league. My affixed signature below indicates that I have read and understand the Player/Parent Code of Conduct and have shared this information with my child so that he might similarly understand exactly what is expected of him.
Ambush Disclaimer and Waiver of Liability
Ambush Academy, Ambush Basketball Academies, and/or any School District and/or any other facility, coaches and staff do not assume any liability for any personal injury or property damage sustained by an athlete as a result of participation in this activity or while in route to or from any group event or practice. An athlete participating in the program and using the equipment and facilities of Ambush and/or any school district or other business, does so as his/her own risk. Sports are physically demanding in nature and athletes who elect to participate and their parents or guardians who consent to their participation must recognize that serious or even life threatening injuries may occur. The participating athlete and his consenting parents or guardians assume the risk or and full responsibility for any damages or injuries which may occur during or as a result of practice, games, or travel to and from an event, and hereby fully and forever release and discharge Iowa Ambush, coaches, staff, volunteers, and facilities used from any and all claims resulting or arising out of the athletes participation in the program events.
Consent for athletic conditioning, training, and health/medical care and procedures.
I hereby give consent for my child to participate in Ambush’s athletic conditioning and training program and to receive any necessary healthcare treatment including first aid, diagnostic procedures, and medical treatment that may be provided by physicians, nurses, athletic trainers, or other health care providers. The healthcare providers have my permission to release my child’s medical information to other healthcare practitioners. In the event I cannot be reached in a medical emergency, I give permission for my child to be transported to receive necessary treatment. This consent pertains to all practices and events that happen within the Ambush Basketball Academies, Ambush Academy and Ambush, LLC.
Activity Agreement
By agreeing below, I attest that I have read and understand the contents of this document and the extent to which participation in athletics may be dangerous. I also understand that Ambush LLC does not assume liability for any damage for personal injury or property damage as a result of my (in the case of the athlete) or my son’s (in the case of the parents or guardians) participation in the program activities or while en route from any program event or practice. I further attest that I have provided accurate information to permit my child to participate in the Ambush Academy, Ambush Basketball Academies and Ambush LLC.
Waiver
Ambush LLC , sponsors, event staff, volunteers, and other personnel are not responsible for any damages or injuries that may occur.
